SCHEDULING SERVICES
At Southwest Diagnostic Centers of Colorado Springs, LTD (SWDC-CS), we understand the importance of scheduling appointments that provide flexibility for patients with busy life styles. Our hours of operation are 7:00 am - 5:00 pm Monday through Friday. Evening and Saturday appointments are also available upon request. For quick and convenient scheduling, please call our direct scheduling line at (719) 219-0789 or fax scheduling requests to (719) 219-2389. The main number of our imaging center is (719) 380-7210 and the main fax number is (719) 380-7510. Our entire staff is focused on ensuring that every patient receives personalized and caring service.

REFERRING OFFICES SCHEDULING INSTRUCTIONS
- We will provide you with our referral forms or prescriptions may be faxed as a mode of referral request. If a prescription is faxed please include the following:
- Home or work phone number for the patient
- Date of birth, social security number
- Diagnosis and any additional instruction regarding the procedure requested
- We accept telephone or fax referrals. Our referral form and patient information sheets are available for printing on our web site.
- If the patient requires same day treatment, please call our office to schedule their appointment.
PATIENT SCHEDULING INSTRUCTIONS
Patients are welcome to schedule appointments at their convenience. We request that patients bring complete insurance information at the time of service. This will increase the efficiency of the diagnostic imaging and reporting process.
Information needed:
- Insurance Card
- Workmen's compensation claim number, adjustor's name and telephone number
- MVA insurance policy number, claim number, adjustor's name and telephone number
- Prescription from your referring doctor
